Nose Piercing Troubles :(

Why do i have an extra piece of skin that is fresh, it has come up next to my nose pericing on the surface and it sticks to my piercing when i bathe it in sea salt water, then it starts to bleed, what do i do? i got my piercing just under one month ago and after i had it for 3 days the piercing fell out so i had to put it back in, is that what has effected it?
